    Which of the following could be classified as a break in

    confidentiality?

    A. A computer hacker breaking into a hospital’s secured

    computer system

    B. Release of medical records from one hospital to another for

    continued patient care

    C. A hospital and physician’s office sharing patient information

    for treatment purposes

    D. A patient discussing his or her medical record with the

    correspondence manager

    I'd go A. D is fine as long as the records are their own. B and C are common practices for patient care as long as the pt gives their permission or it's an emergency. The hacker could be anyone.

    IF the patient has signed a form agreeing to the disclosure between hospitals / MD offices then both B and C are NOT a HIPAA violation. The patient is not forceably made to surrender the information in D so it too is NOT a violation. SO the answer will be A. BUT in truth since it's not noted that the patient signed a disclosure form then A,B, and C would all be breaches in confidentiality.
